Schedule Change Request Form
Students who wish to change their schedule at the beginning of the year, or at the beginning of the 2nd semester, are required to obtain their parent/guardian's signature on the official Course/Schedule Change Request Form.  

When the student has completed the form, they should complete the Google Request Form to see their counselor. Once they meet with their counselor the change to their schedule will be official.
